3/30/2023 EnchantaVaughn Approved Behavior: Client was appropriately dressed for the day. During the session Client was alert and focused. Client’s behavior was acceptable as he was respectful and cooperated with the counselor when prompted or redirected. Incidents of recent anger outbursts by Client were processed and alternative, adaptive ways to express that anger were reviewed. Client is doing a much better job utilizing his therapeutic journal to manage his anger and get back on track. Client verbalizes positive expressions during his therapy sessions. Behavior: Client was supported for acknowledging his mistakes and managing his anxiety on his own. Client transitioned into the next activity without conflict and proceeded to engage into meaningful conversation. Client is aware that the sessions are for him to deescalate unwanted feelings and to challenge himself to overcome symptoms of ADHD. Intervention: QMHP encouraged client to stay in the house and do something special with his family. QMHP encouraged client to be the example. QMHP encouraged client not to follow others that he sees are doing wrong. QMHP intervened to assist in explaining behaviors to client. QMHP reflected with client on his morning, both happy and frustrating experiences. QMHP encouraged client that tomorrow is a new day. QMHP processed client current feelings. QMHP engaged in active listening. QMHP employed positive reinforcement when Client was able to achieve all of his objectives and encouraged him to maintain his positive behavior by providing him with a review of his objectives. Intervention: QMHP introduced concept of nonverbal communication cues in efforts to review the expected behaviors or results to the nonverbal cues. QMHP held consultations with client and father about effective intervention strategies that build on Client's strengths and compensate for his negative behaviors. QMHP encouraged client’s father to provide frequent positive reinforcement to maintain client's interest and motivation in maintaining positive home behaviors and completing his individual home tasks. QMHP implemented positive reward system, as well as, a consequence system. QMHP encouraged client to continue having a good day. Response: Client retained a lot of the information he was taught which motivates him to pay more attention and take his time. Client is observed problem solving on his own and handling his emotions appropriately. Client avoids meltdowns and lashing out when upset or frustrated. Client responds well to feedback. Client responds well to the positive reinforcement provided by QMHP and is seen increasing self-esteem and confidence. Client said he was going to practice coping strategies and relaxation techniques in all aspects of his daily living routine. Client was able to reduce physical overactivity and become more attentive in the session. Response: Client implemented appropriate social interaction skills. Client displayed good study and organizational skills. Client displayed an understanding of ADHD and its symptoms and causes. Client reflected on yesterday’s events. Client smiled and said thank you. Client appeared to be learning from his mistakes and correcting them throughout the day. Client responded well to praise. Client was receptive as QMHP demonstrated and defined the meaning of empathy. Client actively participated with QMHP as they discussed and processed empathy. Client demonstrated that he was able to process productive expressions of anger. Client displayed active behaviors in home setting.
